From person-centered discovery to person-centered planning – this 1-day interactive seminar breaks it all down for human service organizations. While some agencies attempt to understand what really matters to someone they support, they often struggle in how to accurately and consistently gather that information. When it gets to the stage of ensuring that the information is used to directly inform that person’s Life Plan, there can be a disparity between what the organization believes are the highest priority outcomes, and what is truly most important to the person at that individual level.

In this seminar, you will find out how the Personal Outcome Measures® can be used as the foundation for planning. CQL will go well beyond an introduction of the tool, and demonstrate how organizations can use it to empower people in developing their Life Plan, designing supports, achieving outcomes, and tracking progress. Attendees will learn about real-world examples of how other organizations have been able to make it all happen. CQL will also share techniques and tips to improving accessibility of both the discovery process and the plan itself. This offers an opportunity for providers to advocate for people to create their own Life Plan using Personal Outcome Measures® prior to the Life Plan meeting and have the person present it at their meeting, or support them in presenting it.
